It's not a difficult concept to grasp. The majority of living entities are born as either male or female which; when it comes to humans, is usually determined by the type of genitalia they are endowed with. There are exceptions but they are few.
How people then choose to present themselves is not the issue when it comes to determining gender (male or female) and filling out the paperwork at birth. Sorry.
