Armed and trained police officers hiding behind their cars outside a school while the children inside the school were being slaughtered by a mass shooter.

When confronted the response was "oops" we made a mistake. That's it? No accountability for the poor leadership by the higher ups? No serious consequences for those staff level people who obviously failed to do their jobs? Not even so much as reprimands or apologies to the victims?
